| IMPORTANT INFORMATION: |

Before you can use our online system, you must file a
completed Online
Account Application Form with our Administration Office at 546 Dutra Street or the Monterey
Sports Center at 301 E. Franklin Street. You may use the
GUEST option to browse the online activity information without
activating an online account. |
We have posted the
Online Account
Application Form
in an Adobe Acrobat PDF file format. If you don't have the FREE Adobe Acrobat Reader, you can obtain it from Adobe by
clicking on the linked Acrobat icon to the right.

We do not activate online accounts via the telephone
or Internet. Proof of Monterey residency (e.g. copy of drivers license or
household utility bill), if applicable, will be verified when you submit your form. Monterey
City residents who mail-in their forms must include proof of residency.

Upon receipt of your Online Account
Application Form, we will email you a temporary User Name and
Password. The first time you use our online system, you will be asked to establish a
permanent User Name and Password. Once you establish your permanent Password, keep it in a
safe place. Do not share your Password with anyone.

Important Information Regarding Online Registrations |

Always review your shopping cart carefully before
processing your online registration. You are responsible for your online enrollments.
If a correction is required, please contact us BEFORE the first class to request a
transfer or refund. Please refer to our Refund/Transfer policies
listed below. |

Your
printed receipt is your confirmation that the transaction completed successfully and
confirms the status of ENROLLED or WAIT LIST. Please remember to
print your receipt. If you believe your registration did not
process correctly, please call us at 831.646.3866 to confirm your registration. |
|
Don't forget to sign out at the end
of your online session! Failure to sign out will lock your account
and prevent access for standard (not online) registrations. |
|
When enrolling online, only MasterCard, Visa, and American
Express credit cards will be accepted. We do not accept credit card payments or process
registration over the telephone. |
|
Transfers: Transfers must be requested BEFORE the first
class. If space is available, a $2.00 transfer fee will be charged.
Transfer fees must be paid prior to processing. Transfers cannot be
processed using the Online Activity Registration System. |
|
Refunds: No refunds are given after class begins.
If requested BEFORE class begins, refunds are assessed a 25% service charge. Class fees are
not pro-rated. Full refunds
are given if the Department cancels the class. Refunds are not given in cash or credit
card charge-backs; the City will issue a check. Please allow 2 weeks for the refund to
reach you. |
